Dear User

This document contains instructions about the activation of your center in Hypernet (i.e. for the electronic CRF) and consequent password procedures for the users who need an access to the eCRF.

1. Packaging. Hyperphar Group has provided (or is going to provide) your center with a folder containing
a) a "Center activation in Hypernet and password procedures" document (equivalent to this web page)
b) An envelope for each person of the center staff (for which we received a request to create a Hypernet account).
c) In each envelope, which is strictly personal, there is a "USER IDENTIFICATION FORM" and a separate page containing the password

2. Activation of the center. To activate your center we need that
a) you followed the instructions you received from the Victor Project Manager (Wolfgang Bernhardt), including
b) the normal range and unit form which you have signed and handed to your Roche representative (who will fax it to Hyperphar Milan). Form and instructions are in section 9.3 in the S.P.A.M.

3. Activation of users. After the activation of your center we can start with the activation of users.
For each user we need
a) Via fax, a signed copy of the "USER IDENTIFICATION FORM". Instructions and fax number are on the form. Please fill as clearly as possible using capital letters.

4. Notification of activation. We will notify activation to you by the email addresses you wrote in the "USER IDENTIFICATION FORM" (and/or the ones available in our database).

After that you will be able to connect to the Victor project in Hypernet, available at

http://www.hyperphar.com/victor/

5. Password problems???.
a) If you lost or forgot your password, please contact as soon as possible the Victor Project Manager.
b) In case you are experiencing problems with the login, please be sure that your password is active checking that you received our confirmation.
c) If you are an active users and are still having troubles with userID or password, please check exactly how they are typed (userIDs, for example, are all made of 8 digits).
